If the jewelry or other specified item has been lost or stolen, report the theft within 24 hours and obtain a SAP case number
Please contact us on 011-955-6505 for a claim form (Remember that the insured is the name the policy appears in, if the policy is issued in the company name, the company details must be reflected in the block for the insured)
Attach the following details:If the specified item is lost or stolen –
Proof of purchase / a copy of the invoice / a recent valuation for the piece of jewelry or specified item
A quotation for the replacement of the item
If The Specified Item Is Damaged
A damage report stating what caused the damage and the cost of the repairs for the same.
If the item is uneconomical to repair, please also attach the report from the repairer stating the same and a quotation for a replacement.

Please keep the salvage in case the insurance company requires the same
Email the claim form, copies of invoices and items requested above where applicable to your portfolio manager.
Once we have all the required documentation, depending on the value of the claim, an assessor will be appointed who will contact you directly to assess the claim if need be.
Once we have received all the outstanding documents, your claim processed for finalization.
